A form template saves wording and questions that your school uses repeatedly. Apply the template when creating a form, then adjust it for that occasion.
A template is not a form sent to families. Publishing a template makes it available to staff in the dashboard’s Forms area. It does not publish a form in the school app.
In this guide
1. Create a template
- Open Main dashboard → Forms → Form Templates on your dashboard.
- Click Add.
- Enter a clear Title and the template wording in Description.
- Click Add Question and add the questions you need.
- Use Preview to check the wording and test the answer options.
- Click Submit when the template is ready.
2. Choose question options
- Multiple Choice: Provide answer choices and select the response control you need, such as Radio Button.
- Checkboxes: Allow more than one answer.
- Short Answer: Let the user type an answer.
- Enable Comment: Allow a comment alongside the answer.
- Comment Mandatory: Require a comment before the form can be submitted.
- Add option: Add another answer choice, such as Yes or No.
3. Edit a template
Find the template in the list, click its Edit icon, make your changes and click Submit.
4. Publish, unpublish or delete a template
- Publish: Select the template and click Publish to make it available for staff to use when creating forms.
- Unpublish: Select the template and click Unpublish to stop it being used for new forms.
- Delete: Select the template and click Delete only when it is no longer needed.
Unpublishing a template does not unpublish forms already created from it. Manage those forms separately in Forms → Consent Forms & Surveys.
