Check Form Responses in the Administrator App

View assigned forms, consent responses and submitted medical information.

Open Consent Forms in the Administrator App to review responses to forms assigned to you, including consent and submitted medical information needed for school activities.

Can't see a form? Staff access is assigned when the form is set up on the web Dashboard. Ask the person managing the form to check that you have been added.

1. Open your assigned forms

  1. Open the Administrator's App.
  2. Open the main menu: on Android, tap the three horizontal lines at the top left; on iPhone, tap More at the bottom right.
  3. Tap Consent Forms.
  4. Tap the form you want to check.

The forms list shows responses Received, responses still Due and those with consent given (Permitted).

2. Check responses and medical information

Within a form, you can preview its content, check the last submission date, and filter students by Received, Not Received or All.

The medical-information icon beside a student shows:

  • Green: a medical form was submitted with no medical condition reported.
  • Red: a medical form was submitted with a medical condition reported. Tap the icon to view the details.
  • Grey: the medical form has not been submitted.

Check the information, not just the colour. An icon reflects the submitted form; it is not a medical assessment. Review the details you need for the activity and follow school procedures.

3. Follow up on missing responses

Tap Not Received to see students whose forms have not been submitted.

The rectangles beside each student show whether their parents or guardians use the app:

  • Green: the parent or guardian is an app user.
  • Red: the parent or guardian is not an app user.

Check both primary and secondary contacts when deciding how to follow up.

4. Read a completed form

Tap the name of a student whose form has been submitted to view the parent's or guardian's responses for that student.

For setup and staff access, see Create a new form.
